Projections for XPOVIO's U.S. sales in 2026 are now set between $115 million and $130 million. This forecast highlights the increasing competitive pressures within the multiple myeloma treatment sector, which could impede the drug's revenue growth. The market is becoming increasingly crowded, demanding innovative strategies for sustained growth.

Further complicating Karyopharm's trajectory, the U.S. Food and Drug Administration (FDA) issued a Refusal to File letter for the supplemental New Drug Application (sNDA) of selinexor in endometrial cancer. Despite this setback, the company is actively pursuing a new Phase 3 clinical trial focusing on patients with TP53 wild-type endometrial cancer. Topline data from this pivotal trial is anticipated by mid-2026, offering a potential pathway for future regulatory approval.

The Phase 3 SENTRY trial, evaluating selinexor for myelofibrosis, presented mixed outcomes. While the trial successfully met its SVR35 endpoint (Spleen Volume Reduction of 35%), it unfortunately did not achieve the desired symptom score endpoint. The regulatory path for selinexor in myelofibrosis remains ambiguous, although a discernible trend in overall survival (OS) could potentially enhance its prospects.
